User instructions

User instructions for ConsentEASE 1.0 and ConsentEASE 2.0

System Requirements:

Computer/Processor: IBM PC compatible with 486-66 processor, Pentium 166 MHZ or higher recommended.
Operating System: Windows 98 or higher.
Display: SVGA color monitor.
Memory: 8 MB of RAM; additional memory improves performance.
Hard Disk: Minimum 10 MB of free hard disk space.
Drive: CD-ROM Drive.

Installation Instructions:
Insert the CD-ROM disc in your CD-ROM drive.
If auto-run is enabled-
3. You should see ConsentEASE Setup dialog box. Follow the directions on the screen.

4. If no setup dialog box appears, auto-run is disabled. See instructions below.
If auto-run is disabled-
3. Click the Start button,
4. Click Run
3. Enter D:\autorun.exe (where D is the letter of your CD-ROM drive) and click OK.
4. Follow step 1 from above.

User Instructions:
To start the program double click on the ConsentEASE icon on the desktop or-
Click on the Start button,
Choose Programs,
Click on ConsentEASE
Select application

Enter or select the facility you would like to appear on consent. Use Add/Delete button to update the facility name.


Enter or select a patient’s name. Last ten names are saved once you click on the print button. YOU MUST SELECT OR ENTER A PATIENT’S NAME.

Enter or select a doctor’s name you would like to appear on consent. Use Add/Delete button to update the doctor’s names.

Select a specialty. Use add/Delete button to add, delete or modify the specialty and the procedures.


Select the procedure by checking the boxes. YOU CAN SELECT UPTO TEN PROCEDURES. Use Add/Delete button to add, delete or modify the procedures. YOU MUST SELECT A PROCEDURE.

Use print button to
print the consent.

If person signing the consent is other than the patient select the relationship you would like to appear on the consent. Use add/Delete button to modify the description.

Click in the date box to enter a new date. Highlight the time to enter a new time. You must enter the new time in the format as it appears in the box.

Use preview button to preview the consent before printing. Use modify consent button to modify the consent as needed. Use help button for help. Use quit button to close the application.

User instructions for ConsentEASE 3.0 and ConsentEASE 4.0

System Requirements:

Choose the type of consent you would like to print by checking the box. You can check one or more boxes. Surgery box is active when you start the application. If you do not wish to print routine OBGYN surgery consent you can uncheck this box.

Select the procedure by checking the boxes. YOU CAN SELECT UPTO TEN PROCEDURES. Use Add/Delete button to add, delete or modify the procedures.
YOU MUST SELECT A PROCEDURE.

Use print button to print the consent.
